Midwifery Practice and BPJS Health 2025: New Opportunities in the Digital Era

In the midst of the transformation of the national health system, midwifery practice is now entering a new era that is more open, fair and digital. A series of recent regulations from the government and BPJS Kesehatan mark an important momentum for midwives—especially those running independent practices—to become more involved in the National Health Insurance (JKN) system. From changes to maternal and child service regulations to single inpatient class policies, opportunities for collaboration are expanding.

New Regulations, More Open Access

One of the important regulations is Minister of Health Regulation No. 3 of 2023 which stipulates that midwifery services such as antenatal care, childbirth, postnatal care, and pre-referral complications are now included in non-capitation services guaranteed by BPJS Kesehatan. This means that midwives who are members of the FKTP (First Level Health Facility) network are now entitled to provide these services at a rate determined and directly covered by BPJS.

Even farther, Presidential Decree No. 59 of 2024 establishes the implementation of the Standard Inpatient Class (KRIS) which will come into effect on July 1, 2025. This policy eliminates differences in services based on contribution class (Class 1–3) and replaces them with one service standard, so that all participants receive equal services—including pregnant women and newborns.

What are the Benefits for Midwifery Practice?

  1. More Guaranteed Service

Midwifery practices integrated with FKTP will have easier access to non-capitation rates for core services such as pregnancy and childbirth. This helps midwives focus on providing the best service without worrying about excessive administrative burdens.

  1. Patients Are More Comfortable and Protected

With a single inpatient class, patients are no longer divided based on their ability to pay. Pregnant women in rural and urban areas have the same right to services—an important step in realizing health equity.

  1. A Fairer Contribution Model

Starting July 2025, BPJS contribution system will be simplified, referring to the economic conditions of the participants. However, basic services are still guaranteed—providing certainty for all parties.

  1. Integration of Independent Practice into BPJS System

Now, independent midwife practices can be more easily connected to the BPJS system through collaboration with health centers or clinics. Although licensing and administrative processes are still needed, this step opens up great opportunities to expand the reach of community-based midwifery services.
